The Intensive Home Nursing service (IHNS) was established in 1995 and is a service that provides care for people who are in the last days/ to weeks of life across the city of Sheffield, enabling them to die at home
and to prevent hospital admissions.
The care and support offered by the service is available 24 hours a day, 7 days per week including bank holidays. The core office hours are 8-5pm, 7 days per week.
The service delivers personal care, support with medication, emotional support and enables family carers to have some respite from their caring responsibilities.
The IHNS works in close partnership with Continuing Healthcare (CHC), Community Nursing and Palliative care providers to support patients who have been Fast tracked.
All referrals that made to the service are completed by a Health Care Professional.
Main duties of the job
•To provide operational management of the Intensive Home Nursing Service (IHNS) to ensure the
service delivers high quality, coordinated, patient centred and effective care.
• To provide specialist input for planning, development and evaluation of all services provided within the Intensive Home Nursing service, holding responsibility for defined projects.
• To provide professional leadership for the IHNS team to include recruitment, supervision, and development of these staff.
• To provide clinical leadership and management to a designated staff group and take the responsibility for the dissemination of evidence-based practice across the service.
• To contribute, develop and implement service changes for the service supporting the strategic direction of the service.
This vacancy is advertised on a fixed term/secondment contract basis for a maximum period of six months due to covering employee absence. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts and secondments in relation to the substantive post holder.
Please also note any secondment must first be agreed with your current line manager.

Essential criteria
1. RGN Level One
2. NMC Registered
3. To have completed the national practice assessor training or equivalent teaching and assessing qualification for those with ENB998.
4. Continuous Professional Development

5. First degree or evidence of working towards one (demonstrating graduate attributes)
Skills and Knowledge
Essential criteria
6. Car Driver
7. Palliative Care skills and knowledge across a range of patient care pathways
8. Demonstration of compassionate and inclusive leadership
9. Persuasion and influencing skills – ability to present ideas and proposals effectively.
Experience
Essential criteria
10. Extensive post registration experience across a range of patient conditions
11. Experience of leading change and implementing service improvements
12. Relevant experience at junior sister/ charge nurse role
13. Ability to lead, motivate and develop a professional team
Desirable criteria
14. Experience and knowledge of Palliative Care and End of Life approaches and care
15. Academic courses relevant to role

16. Ability to communicate effectively including active listening, writing, and verbally, with individuals on a one-toone basis and with motivational skills
