Job Title: Social Worker - Mental Health - Kingston Upon Thames
Pay Rate: £31.31 per hour
Liquid Personnel is currently seeking an experienced Adult Social Worker to join the Mental Health Team of our client. We are looking for a qualified social worker with experience in mental health to review supported living placements, with the majority of reviews conducted jointly with Health partners.
Responsibilities
* Evaluate patients' mental health status, identifying needs, strengths, and areas for intervention.
* Collaborate with the healthcare team to create individualized treatment plans that address patients' mental health needs and goals.
* Respond to mental health crises, providing immediate support and intervention to stabilize patients.
* Attend interdisciplinary team meetings and case conferences to discuss patient care and collaborate with other healthcare professionals.

* Social Work England registration
