As a Leaving Care Worker, you will play a vital role in supporting young people aged 16–25 who are transitioning from care to independent living. You will act as a Lead Professional, building strong relationships and delivering tailored support plans that empower care leavers to achieve their goals. DUTIES & RESPONSIBILITIES INCLUDE: Caseload Management: Hold responsibility for a defined caseload of care leavers, ensuring timely assessments, planning, and interventions. Pathway Planning: Develop and review aspirational Pathway Plans covering accommodation, education, employment, health, and financial independence. Practical Support: Assist with budgeting, bills, tenancy management, shopping, and life-skills training. Advocacy & Coordination: Work collaboratively with multi-agency partners (housing, education, health, benefits) to secure positive outcomes.
