The post holder will be a key member of the Integrated Neighbourhood teams, responsible for case managing complex patients who are homeless or at risk of becoming homeless. They will ensure the smooth operation of coordinating multi-disciplinary team meetings and work as an autonomous practitioner within their specialty area.
The post holder will act as the link between primary care and key core provider services within the locality, working with General Practitioners, practice staff, community services from ELHT, Advanced Nurse Practitioners, specialist services, and partner agencies to build high-impact partnerships and drive transformation.
They will develop and promote a case management approach to care, emphasizing coordinated, holistic patient care, facilitated through the organization and planning of multi-disciplinary team meetings. Appropriate training in holistic care planning and delivering patient-centered services is required.
The role involves providing case management for patients with complex needs in the community, especially those experiencing homelessness or at risk. The post holder will work within the community, assessing patients in their environments to achieve optimal outcomes, and will collaborate across divisions with GPs and partner agencies to facilitate earlier community care with positive results.
The post holder will provide leadership and direction for effective service delivery, support the manager in implementing strategic initiatives, and aim to reduce avoidable hospital admissions while maintaining high clinical standards.
