The Young Adult Transition Team have an exciting opportunity for a Social Care Officer to join our dynamic and dedicated team which provides support and social care provision in Hartlepool to young people and adults with Care Act eligible needs.
As a team member, you'll play a vital role in supporting young people aged 16 to 25 as they move into adulthood, helping them navigate complex needs and access the right support at the right time. You'll be part of a warm, collaborative, and forward thinking team, working alongside a wide network of professionals including CAMHS, SEND, Children's Services, Re-ablement, Mental Health Teams and more.
Your responsibilities will include completing annual reviews and social care assessments of health and social care provision regarding commissioned services and direct payments. You will also need good administrative skills and proven experience of developing, maintaining and inputting information on electronic-based systems in order to provide timely management information.
The successful applicant will receive regular supervision and an annual appraisal; they will be well supported and have opportunities in respect of their career development with the potential to experience a variety of roles within adult social care.
Hartlepool Borough Council is committed to safeguarding and promoting the welfare of children and vulnerable adults. The successful applicant will be required to undertake a DBS check via the Disclosure and Barring Service.
To be successful in this role, you will have recent relevant experience of working in a Social Care setting and of dealing with enquiries from members of the public or other agencies. A sound knowledge base and understanding of roles and responsibilities within Social Care is essential, ideally including knowledge of current developments and service pathways.
