We have a vacancy in the Spalding Older Adult OACMHT and are seeking to recruit a Band 6 Community Psychiatric Nurse to work within our team. The team consists of CPNS, associate practitioners and a nurse associate. This post could also be a development post; therefore, it would be suitable for a newly qualified member of staff at Band 5 who could be supported through a competency framework to support transition to a senior Band 6 CPN.
The Team provides a service across Monday to Friday 9-5, offering assessment, diagnosis, and treatment for patients of all ages presenting with memory difficulties and for older adults experiencing mental health difficulties. The successful applicant will be providing high quality, person-centred care to patients, families, and carers and working as part of the MDT to ensure good patient outcomes.
Liaising with various stakeholders will be a regular part of your job; this can include GP surgeries, carers and families, other teams within LPFT, and Adult Social Care. Regular joint working with other teams within LPFT, such as the Home Treatment Teams and the inpatient wards, requires some flexibility and shared responsibility.
Our division has an integrated home treatment team to support patients who may be requiring enhanced support.
The team is based at the Johnson Community Hospital site, but the applicant will be expected to work across the surrounding area.
To provide a high standard of clinical care, ensuring safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools.
To delegate aspects of the patient treatment plan to other members of the team, whilst maintaining overall responsibility for patients care, where appropriate.
To ensure the clinical caseload and clinical practice of nursing members of the Community Mental Health Team are of the highest standard of clinical care.
To manage case-load and service delivery in accordance with and in order to contribute towards the attainment of Service/Trust productivity requirements.
To act as care co-ordinator under the Care Programme Approach, ensuring up-to-date agreed care plans, risk assessments and reviews are in place.
Work with individuals to assess their mental health, recognise mental illness and identify their related needs and circumstances, and enable them to understand, manage and, where appropriate, change their behaviour.
Plan, implement, review and improve interventions to meet people’s identified needs and manage their inherent risk.
To assess carers’ and families needs and develop, implement and review programmes of support for carers and families.
Protect people from abuse, neglect and harm.
Enable individuals to develop independent living skills and live in the community.
