This is an exciting opportunity todevelop your career as a Pharmacy Technician within our well established and highly skilledpractice employed pharmacy team. This vacancy has arisen following theresignation of our current Pharmacy Technician who is leaving to pursue aslightly different role outside primary care.

The post wouldbe based at Air Balloon Surgery, although funded through the FABB PCN AdditionRoles Reimbursement Scheme.

The post wouldsuit a GPhC registered pharmacy technician who wishes to progress their careerin a knowledgeable and supportive environment within primary care, and who haseither enrolled in or has qualified through an approved pathway such as thePrimary Care Pharmacy Educational Pathway(PCPEP) or Medicines Optimisation in Care Homes (MOCH) or is willing toundertake such training.


Main duties of the job

The successfulpost holder will work with our three Practice Clinical Pharmacists to undertake patientfacing and patientsupporting roles to ensureeffective use of medications, including medicines optimisation tasks, andsupporting medication reviews.

Hours

Ideallythe post holder would work full time over five days, Monday to Friday, but wewould be happy to consider a part-time role for the right candidate.

Pay and conditions

Payto be discussed, dependent on experience.Includes NHS Pension Scheme and 5.6 weeks pro-rata paid holiday, notincluding bank holidays.

Job responsibilities

JobSummary:

To work under thesupervision of the Practice Pharmacists to ensure the safe, accurate andtimely supply of prescriptions to patients.

To provide technical andadministrative support to the Clinical Pharmacists and Clinicians.

Support the Care Navigatorteam with complex queries around medication from patients.

Completeaudits around patient safety and cost-effective prescribing and present theinformation for sharing with the clinical team.

Clinical responsibilities

a.Undertake patient facing and patientsupporting roles to ensure effective medicines use, through shareddecision-making conversations with patients.

b.Carry out medicines optimisation tasks includingeffective medicine administration (e.g. checking inhaler technique), supportingmedication reviews, and medicines reconciliation. Where required,utilise consultation skillsto work in partnership with patients to ensure they use their medicineseffectively.

c.Support medication reviews and medicinesreconciliation for new care home patients and synchronising medicinesfor patient transfers between care settings and linking with localcommunity pharmacists.

d.Provide specialist expertise, where competent, toaddress both the public health and socialcare needs of patients, includinglifestyle advice, service information, and help in tacklinglocal health inequalities.

e.Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing.

f.Assist in the delivery of medicines optimisation and management incentive schemes and patient safetyaudits.

g.Support the implementation of national prescribingpolicies and guidance. This will be achieved through undertakingclinical audits (e.g. use of antibiotics), supporting quality improvementmeasures and contributing to the Quality and Outcomes Framework and enhancedservices.

Management responsibilities

a.Work with the practicemulti-disciplinary team to ensure efficientmedicines optimisation, including implementing efficient ordering andreturn processes, and reducing wastage.

b.Supervise practice Care Navigation team in sortingand streaming general prescription requests, so as to allow GPs and clinicalpharmacists to review themore clinically complex requests.

c.Support initiatives for medicines optimisation systemsto get the best value from medicines by encouraging and supporting Electronic Prescriptions, saferepeat prescribing systems, and timely monitoring and management of high-riskmedicines.

d.Provide training and support as requested, on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS).

e.Under the supervision of the Senior PracticePharmacist- manage and action MHRA drug alerts, recording actions and workingwithin required time frames.

f.Support the practice to meet local prescribingbudgetary targets and performance schemes.

* Full driving licence


Qualifications

* NVQ Level 3 Diploma in Pharmacy Service Skills (QCF) plus Level 3 QCF Diploma in Pharmaceutical Science or GPhC accredited equivalent prior to July 2011.
* Is registered with the General Pharmaceutical Council (GPhC).
* Meets the specific qualification and training requirements as specified by the GPhC criteria to register as a Pharmacy Technician.
* Enrolled in, undertaking, qualified from, or willing to enrol in and undertake an approved training pathway. For example, the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH).


Experience

* Completion of 2 years consolidation of learning period post qualification
* Broad range of clinical experience
* Able to work independently
* Demonstrates ability to manage clinical risk
* Community or primary care working
* Evidence of clinical evaluation and audit work
* Knowledge of Emis software
