Job Responsibilities
1. The post holder will:
2. Support and deliver care to patients who have been admitted to the Community Health Team caseload and will coordinate with the team to approach care delivery on a day-to-day basis.
3. Assess, plan, and implement a care plan for individual patients.
4. Improve clinical outcomes for patients by enabling them to function independently, increasing their ability to remain in their own home/community, and reducing the need for or preventing inappropriate hospital admissions.
5. Support joint integrated working across the Community Health Team and other stakeholders.
6. Work closely with the Team Senior Nurse/AHP to ensure care delivery is of high quality and meets required standards.
Additional Duties
1. Assess patients and gain their trust.
2. Plan and deliver patients' care.
3. Take samples, pulses, temperatures, and blood pressures.
4. Monitor and administer medication.
