Job overview
We seek an experienced and enthusiastic Band 7 Heart Failure Clinical Nurse Specialist to provide specialist advice and guidance around heart failure care within inpatient and community settings. The role supports multidisciplinary teams, leads discharge planning, and contributes to service improvement.
Main responsibilities
* Provide clinical leadership, advice and support to staff and the multidisciplinary team (MDT) on all aspects of heart failure care.
* Conduct direct clinical assessments, prescribe medication and provide specialist guidance for patients with confirmed heart failure.
* Facilitate early identification and assessment of admitted patients, liaising with GPs, community services and social services.
* Develop and maintain care pathways, discharge planning and patient education materials.
* Act as key resource for knowledge and expertise for patients, relatives and health professionals.
* Share expertise through teaching colleagues and educating patients/carers.
Education and qualifications
* Registered general nurse (RGN) and NMC registered.
* Completed national PEAP programme or equivalent qualification for practice supervisors.
* First degree (or currently studying a relevant degree).
* For Band 7: Non‑medical prescribing (or willing to undertake) and advanced practice and consultation skills (or willing to undertake).
Experience and skills
* Extensive post‑registration experience, ideally within a cardiology/cardiothoracic setting.
* Experience as a heart failure specialist nurse (Band 6 or 7).
* Strong communication, interpersonal, and performance management skills.
* Excellent organisational ability to manage complex workload under pressure and prioritise tasks.
* Ability to work autonomously and within a multidisciplinary team.
* Basic ECG interpretation and cannulation/venepuncture skills.
* Willingness to undertake advanced ECG interpretation where not presently held.
