A new exiting opportunity has arisen within Northenden and Brooklands Primary Care Network (PCN) to recruit a GP Assistant role to assist the GPs and other clinicians in their daily administrative tasks.
This is an interesting new role for someone who is enthusiastic in supporting patients in their health care. This role will help to deliver better outcomes for them by supporting GPs and practices to deliver excellent health care.
Duties will also include some minor clinical tasks. In addition to the Practice level work there will be sessions working in the 3 PCN care homes.
Two homes support people with a learning disability (LD) and the other home supports elderly patients. This support is to achieve better health outcomes and experience of services for the patients and their carers.

The successful candidate should have a Healthcare qualification (level 3 or 4) or working towards gaining an equivalent level/relevant experience.
Main duties of the job
A GPA will support the GP(s) smooth running of clinics byperforming the more routine administration and clinical tasks on behalf of theGP. This in turn will free up the GPstime to focus on the patient within a practice setting, resulting in improvedpatient flow and experience, and increasing the time efficiency ofappointments.
Supporting the 80 approx. patients residing in the care homes within the PCN. This will involve assisting with ward rounds and arranging any necessary follow ups, tests or investigations to achieve better health outcomes and experience of services.
This role is based within GP Practices and care homes. The role is intendedto become a part of multidisciplinary teams to further embed a personalisedcare approach.

Northenden and Brooklands Primary Care Network consists of 5 practices serving almost 30,000 patients.
The job will involve supporting the 5 practices which are:
Brooklands, The Park, Northern Moor, Northenden and Woodlands Medical Practices which are all within a mile 1.5 mile radius of each other.
Job responsibilities
Main duties to include :
Care home work :
Will work with GPs and practice staff toorganise weekly care home rounds.
This will include information gathering ,proactive reviews, taking clinical observations, completing referrals andchecking re resident appointments to help complete care planning.
Liaise with the care homes on a regular basis and liaise with GPpractices if required.
Support the GPs to carry out new resident andproactive reviews.
Help deal with the daily queries received fromthe care homes, seeing the queries through to completion and linking in withthe relevant clinicians to enable this completion.
Signpost practice team members, service usersand carers to relevant services.
Liaise with other stakeholders as needed forthe collective benefit of patients including but not limited to patients, carers,GPs, Nurses, other practice staff and other healthcare professionals.
Develop excellent working relationships withall care home staff, GP staff, wider service networks including the voluntarysector, adult social care, hospitals, community pharmacists and other membersof the MDT.
Providinga point of contact for both staff andpatients in coordinating care and will help to deal with incoming queries.
Support patients to access personalised careand support plans, in line with best practice.
Ensure regular and consistent communication withcare homes regarding patient progress and any complications, includingsupporting the coordination of ward rounds.
Extracting information from clinical lettersthat needs coding and adding to notes.
Arranging appointments, referrals, tests andfollow up appointments of patients.
Preparing patients prior to going in to see theGP, taking a brief history and basic readings in readiness for the GPappointment.
Completing basic (non-opinion) forms and coreelements of some forms for the GP to approve and sign such as insurance forms,mortgage, benefits agency forms etc GPA Competency Framework 2.
Explaining treatment procedures to patients.
Helping the GP liaise with outside agencies e.g.getting an on-call doctor on the phone to ask advice or arrange admission whilethe GP can continue with their consultation(s).

* Experience of administration in a health care setting

Qualifications

* Computer literate with the ability to use Microsoft Office Programs such as Word and Excel or equivalent experience
* Qualified in Phlebotomy
