An exciting opportunity has arisen for a full-time/ Part time Band 6 Respiratory Nurse Specialist in the well-established Leeds Community Respiratory Team. The Leeds Community Respiratory Team is made up of Specialist Nurses, Physiotherapists, HOS-AR associate practitioners and clinical support workers providing care and treatment for patients with COPD and other Respiratory disease, supporting admission avoidance, chronic disease management, pulmonary rehabilitation ,Home ward Respiratory and the home oxygen service.
Main duties of the job
You will provide care to patients predominantly with Chronic Obstructive Pulmonary Disease (COPD) through different pathways, such as home visits or Clinics for hospital discharge as well as chronic disease management.
The team run the Home Oxygen Service in Leeds which provides assessment and monitoring for respiratory and non-respiratory patients requiring home oxygen.
We are looking for passionate, dynamic and flexible individuals with some experience of managing respiratory patients in a community or hospital setting.
The individual is required to adapt to changing demands and workload to continue to deliver high quality care and demonstrate clinical decision making working independently or as part of the wider multi-disciplinary team.
The respiratory service is a 7day service operating from 8:30am to 4:30pm, due to the nature of the role, it is essential that you are able to travel independently across the city and have access to a suitable vehicle for business purposes.

The post holder will be responsible for their own caseload within their speciality area as an autonomous practitioner. In doing so, they will assess, plan, deliver and evaluate patient centred programmes of care to a specialist level ensuring that they practice within their sphere of competence and knowledge.
