* Pay Rate: £38 per hour (via umbrella)

About the Role:
Somerset is seeking a dedicated Acute Hospital Social Worker to join our team, supporting hospital discharges in line with the intermediate care process. This is an on‑site role, requiring daily attendance at the hospital to ensure smooth, timely discharges and safe transitions for patients.
Key Responsibilities:
* Support hospital discharges in partnership with clinical teams and community services.
* Undertake assessments and care planning in line with the Care Act and intermediate care processes.
* Apply knowledge of the Mental Capacity Act to support decision‑making and best interests planning.
* Work collaboratively with multi‑agency teams to promote safe, effective patient flow.
* Provide advice, guidance, and signposting to patients and families to support independent living or ongoing care.
