Please refer to the attached JD, Summary of key responsibilities: Consult with patients and their families to undertake assessments of patient need to support them in managing long term conditions and improve patient health outcomes locally and nationally. Consult with patients day to day and provide routine nursing care to patient. Work with teams across the practice, primary care network and wider neighbourhood teams. Contribute to safe working, ensuring practice protocols are followed and updated when required.
