Advanced Clinical Practitioner £49,000 to £65,250 a year plus rate enhancements for evening & weekend working

gtd healthcare is one of the largest NHS commissioned Primary and Urgent care providers in North West England. We have an excellent opportunity for an Advanced Clinical Practitioner to join our Urgent Care Teams at Royal Preston Hospital and Chorley Hospital.
Here at Preston and Chorley Integrated Urgent Care Centre in the heart of Lancashire, we provide varied service lines, which include, DVT clinics, minor injuries, face to face walk in patients, NHS 111 appointments, remote clinical assessment calls and have a team of Clinical Practitioners providing the triage functions between the emergency departments and urgent care centres.
Our team is made up of Advanced Clinical Practitioners with a blend of Illness and/or injury training and from a variety of backgrounds; such as Nurses, and Paramedics, where shared learning, continual improvement and delivering excellence is part of our embedded culture.
You will work alongside established Advanced Clinical Practitioners, GP's or middle grade Doctors as well as having a supportive administrative team who help coordinate care and flow of patients.

As an Advanced Clinical Practitioner, your responsibilities and duties will include:
* Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
* Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
* Provide information and advice on prescribed or over-the-counter medication and medication regimes, side-effects and interactions.
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
* Refer and discharge safely ensuring that safety-netting advice is offered.
* Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.
