Job Title: Discharge Planning Nurse/AHP
This exciting position is based within the Fife-wide Integrated Discharge Team, primarily located at Victoria Hospital, Kirkcaldy, with requirements to work across Community Hospitals in Fife. The role involves comprehensive assessment and discharge planning to support timely patient discharge, aligned with the Home First Model and patient-centered pathways.
Responsibilities include attending ward daily multidisciplinary team (MDT) meetings, conducting assessments, and communicating with relevant teams across the Partnership and Third Sector organizations. The ideal candidate will approach complex discharge planning dynamically and have experience within Health and Social Care, particularly with long-term conditions and frailty affecting the aging population.
You must hold a current qualification/registration in Nursing or an Allied Health Professional (AHP) role and be capable of working autonomously, making decisions, and providing updates on discharge pathways and plans. Excellent communication skills, organization, and flexibility are essential.
Knowledge of Microsoft Office and associated databases (e.g., Excel) is required.
