We areseeking to recruit an enthusiastic and experienced Nurse Practitioner/ParamedicPractitioner to join our well-established clinical team, to replace aNurse Practitioner who is retiring. Theideal candidate is an experienced clinician who can provide autonomous care forregistered patients of the practice. He or She should have excellent clinicalskills and will be competent in assessing, diagnosing, planning, implementing,and evaluating treatment/interventions independently and autonomously
Previousexperience in minor illness in a primary care health setting isessential. You must be able to demonstrate safe, clinical decision making forpatients acting within professional boundaries.
You should demonstrate critical thinking in the clinical decision-making process. You should be able to work collaboratively with the general practice team to meet the needs ofpatients, supporting the delivery of policy and procedures, and provideleadership as required.
In order towork at this level, NMC requirements for advanced practice must be met.

Job responsibilities
JOB PURPOSE
To provide an additional / alternative service to patientswith a range of acute conditions, covering a range of specialist areas.
To provide direct access for clients with undifferentiatedand undiagnosed problems and make an assessment of their health care needsbased on highly developed nursing knowledge and skills.
To admit or discharge clients from caseload and refer themto other health care professionals asappropriate.
To make professional clinical decisions within the role ofan autonomous practitioner, applying evidence-based practice and help lead onprovision of nurse access for acute clinical conditions.
To practice within the NMC Code of Professional Conduct andmaintain accurate records.
To provide advice and accept referrals from other healthprofessionals
To accept a share of the daily tasks that are allocated toGPs and NPs on rota system ie, on call duties, home visiting for acute minorailments, acting on electronic lab results and hospital correspondence.
To prescribe from an agreed formulary and refer to GPs asnecessary.
To work collaboratively with members of the PHCT.
To provide therapeutic interventions within a broader healthpromotion/public health strategy and enable clients to take action to meettheir priorities for health.
To help provide specialist education and training regardingacute/chronic clinical conditions to other nursing staff and students.

To provide direct access for patients with undifferentiatedand undiagnosed clinical conditions to the practice population.
To utilise specialist clinical skills to assess, diagnoseand implement management of this specific group of patients incorporatingevidence-based guidance, and encompassing relevant local policies.
To refer patients directly to other health careprofessionals as needed and collaborate with health professionals acrossprimary and secondary care, utilising locally agreed guidelines.
To prescribe from the independent/supplementary nurseprescribing formulary and refer to GPs as necessary.
To communicate effectively with patients and carersutilising appropriate skills e.g. empathy, attentive listening.
Develop and provide autonomous nurse-led service to provideaccess for assessment and management of acute clinical conditions.
Assist with leading the Nursing Team in the planning and implementation of careprovision for the practice population in support of the practice developmentplans.
Understand the importance of recognition, observation,documentation, communication and referral on of child abuse or family violenceand be aware of local guidelines.
To provide health promotion advice opportunistically andspecifically whilst recognising and utilising the cycle of change.
To provide a nurse telephone advice service for the practicepopulation when required.
To assist with the development of clinical protocols for thenursing team.
.Work with other members of the Practice Team to monitor andevaluate care provision.
To assist in clinical audit and the setting and monitoringof standards of care and implement standards for quality assurance.
To act as a specialist adviser/resource/mentor to juniornursing staff, associate nurses and other health care professionals.
Undertake educational teaching sessions in-house to membersof these professionals to support their professional development plans.
To attend educational meeting
To attend monthly nurse and PLT meetings when possible.
To undertake such duties as designated by the GPs withinthe accountability sphere of the NMC.
Person Specification
Qualifications
* Registered General Nurse: Registered with the NMC
* BSc Hons Nurse Practitioner Degree or equivalent qualification, or working towards)
* Experience of working autonomously

* At least 2 years recent primary/community experience
* Relevant diplomas in long term conditions
