Role overview
Renfrewshire HSCP has a diverse population of approximately 180,000 people and employs around 2000 WTE staff. Services managed include Health Visitors, District Nurses, Allied Health Professionals, Addiction, Learning Disability Services, Palliative Care, Local Adult and Older Mental Health Services, Frailty Services.
The Hospital at Home Service is a short‑term, targeted intervention that provides a level of acute hospital care in an individual’s own home that is equivalent to that provided within a hospital. It supports acute hospital admission avoidance and early supported discharges aligned with the frailty pathway.

Key responsibilities
Clinical
* Undertake a comprehensive assessment of frailty positive patients, including screening and multi‑professional baseline assessments, with a complex presentation using investigative skills, analysing clinical and non‑clinical information and providing a functional diagnosis for individual patients to determine their need for Physiotherapy intervention, using formal assessment tools as appropriate.
* Following assessment, act autonomously to assess, develop, implement and evaluate Physiotherapy interventions for a range of patients, including those with highly complex presentations and multi‑factorial health and social needs.
* Provide specialist interventions based on a problem solving and advanced clinical reasoning approach, delivering interventions using a wide range of treatment techniques and monitoring, evaluating and modifying treatment to measure progress and ensure effectiveness.
* Undertake a case coordination role, coordinating interdisciplinary goal planning and discharge planning, liaising and working collaboratively with other agencies, sharing information and participating in case conferences.
* Assess for a range of equipment, such as orthotics and walking aids, providing appropriate equipment and referral onto other specialist services, and manage care and maintenance of equipment.
* Autonomously manage a professional caseload, ensuring efficient workload management and adjustment of service provision to meet changing priorities.
* Be professionally and legally accountable for all aspects of own work, complying with CSP and HCPC guidelines and national and local policies.
* Maintain patient documentation records and accurate statistical information within computerised systems.
* Apply a high level of understanding of the effect of frailty and disability and provide advice and support to patients and their families/carers on lifestyle changes and adaptations to the social and physical environment.
* Provide specialist advice and teaching to multidisciplinary team members, external agencies and families regarding client management to ensure consistent care.
Managerial/Supervision
* Take a coordinating role and be responsible for day‑to‑day management and delivery of Physiotherapy services in conjunction with the interdisciplinary service.
* Delegate caseload, tasks and supervise Physiotherapists, Rehabilitation Support Workers and students, participating in induction and development programmes.
* Contribute to identifying gaps and pressures within the service and discussing remedial action with the Operational Team Leader.
* Actively lead identified areas within clinical effectiveness, such as audits, journal clubs, peer review and projects to support the health care governance agenda.
* Ensure adherence to Health & Safety Policy and maintain a safe working environment for patients and staff.
* Assist the Operational Team Lead in recruitment and selection processes to ensure appropriate appointments and retention of staff.
Educational and Research
* Contribute to in‑service training and provide specialist input to the Service programme to promote personal and professional development.
* Provide specialist teaching to other members of the multidisciplinary service, patients, carers and agencies, including formal and informal sessions for less experienced staff and students.
* Responsible for clinical training, supervision and formal assessment of undergraduate and postgraduate students on placement.
* Carry out own professional development, maintaining a professional portfolio and incorporating evidence into practice.
Qualifications and experience
Essential
* Degree/Diploma or equivalent in Physiotherapy.
* Registered with the relevant Health Professions Council (e.g. HCPC).
* Excellent communication skills.
* Current full driving licence.
Desirable
* Experience at postgraduate level, particularly within Care of the Elderly, Adult Rehabilitation, Neurology, Orthopaedics, Respiratory or Medicine Specialities.
* Ability to work independently.
* Experience working within a multidisciplinary community‑based team.
* Leadership skills.
* Knowledge of IT systems.
* Evidence of continuous professional development.
