Libertatem Healthcare Group is seeking an experienced brain injury support worker to support a male client in Bishop's Stortford. The client is 14 years old and is currently supported by his family.

Hours: Full-Time and Part-Time available. 20:00 - 08:00 (12-hour shift)
Responsibilities:
* Provide personalized support according to the individual’s support plan, maintaining confidentiality and sensitivity.
* Follow care plans and participate in monitoring and review processes.
* Work professionally with multidisciplinary teams and health professionals.
* Promote independence through support with mobility, feeding, hygiene, and dressing.
* Manage neurogenic bladder and bowel care as specified in care plans.
* Assist with domestic tasks, meal preparation, shopping, and appointment attendance.
* Provide emotional support to enhance confidence and motivation.
* Assist with moving and handling equipment to ensure safety and independence.
* Administer medication as per care plans and policies.
* Support rehabilitation activities as directed.
* Respect service users’ cultural and personal backgrounds in all support activities.
* Document progress and changes in service users’ conditions accurately.
Communication:
* Coordinate with healthcare team members and report changes or concerns.
* Report safeguarding issues, incidents, hazards, and feedback in line with company procedures.
* Maintain clear, secure, and confidential records.
* Utilize the ‘On Call’ system for additional support when needed.
Enhanced DBS check and full training will be provided. Support from the clinical lead nurse will be available.
