Qualified Social Worker- Preparing for Adulthood Team
An exciting opportunity has arisen for an experienced Social Worker to join a Preparing for Adulthood team supporting young people aged 14-25 with complex needs as they transition into adulthood.
You'll work collaboratively with families, education, health, and care services to ensure young people have the support they need to achieve independence and positive outcomes.

Responsibilities:
* Support young people with disabilities or additional needs transitioning from Children's to Adults' Services
* Complete Care Act assessments and reviews in line with statutory duties
* Work in partnership with education and health services to develop person-centred transition plans
* Promote independence, choice, and wellbeing in line with strengths-based practiceAttend multi-agency meetings and contribute to EHCP and safeguarding processesBuild strong relationships with families and professionals to deliver coordinated care
To be considered for the role, ideally you have:
* Diploma/Degree in Social Work
* Registration with Social Work England
* Experience working with young people with disabilities, SEND, or complex needs
* Knowledge of the Care Act, Mental Capacity Act, and transition planning
* Ability to work flexibly and manage a varied caseload
