Community Palliative Healthcare Assistant.

Location: Northamptonshire

Working Pattern: Full-time, part-time and bank hours available

8am - 8pm

8pm - 8am

Salary: DHU Band 3, £24,745 per annum (pro rata)

At DHU Healthcare, our Urgent and Emergency Care teams work on the frontline of the NHS, delivering expert, compassionate care 24/7, 365 days a year. From Emergency Departments and Urgent Treatment Centres to GP surgeries and communities across the East Midlands and beyond, we're there when people need us most.

Our teams of skilled medical, nursing, allied health, and support professionals all work together to respond quickly and effectively to every patient. We deliver a wide range of services including community nursing, urgent treatment, primary care streaming, urgent on-day primary care, GP home visiting, phlebotomy, community hospital medical cover, telephone consultation support, and palliative care home visiting. As a Community Palliative Healthcare Assistant, you will: Provide compassionate, person-centred care to individuals at end of life in their own homes.
Support with personal care, comfort, nutrition, hydration, and mobility in line with care plans.
Offer emotional reassurance to patients, families, and carers during a sensitive time.
Observe and report changes in condition to the clinical team to ensure safe, responsive care.
Work respectfully as part of a supportive multidisciplinary team, maintaining dignity, privacy, and high standards of care.
