Responsibilities
* To be professionally and legally accountable for all aspects of your own and delegated work
* To work within codes of practice and professional guidelines
* To undertake holistic assessment (including those with complex presentations and multi pathologies) using specialist clinical reasoning skills
* To use evidence-based practice to ensure clinical care is effective and appropriate
* To promote independence and, wherever possible, to avoid hospital admission and reduce inpatient length of stay
* Organise and manage own time, delegating work appropriately
* Conduct risk assessments and health and safety assessments, reporting any changes/newly identified risks
* Demonstrate the importance of gaining patient and carer consent in all interventions
* Communicate effectively with patients, carers and colleagues, including supporting patients with communication difficulties
* Be actively involved in the development of the service/organisational aims and the new model of care
* Work effectively within the team structure and liaise with the MDT and other agencies by attending MDT meetings and case reviews
* Ensure accurate and timely written records in line with Trust policy and HCPC guidance, reporting issues as appropriate
* Provide specialist advice to all members of the multidisciplinary team and service users on changes involving current care plans and patient progress
* Support day-to-day planning and delivery of high-quality care

Education / Qualifications
Essential criteria
* Degree or equivalent (e.g., BSc, MSc or Diploma) entry-level qualification into the Physiotherapy profession
* Current HCPC registration
* Documented evidence of continuing professional development
* Willingness to undertake Fieldwork/Practice Educator qualification
Desirable criteria
* Member of CSP
* Fieldwork/Practice Educator qualification
Experience
