Job overview

We are seeking a dedicated and compassionate Band 4 Rehabilitation Assistant Practitioner to join the Community Independence Service (CIS) in Hammersmith & Fulham, a collaborative initiative between West London NHS Trust and CNWL Foundation Trust. The CIS delivers both planned and unplanned nursing and therapy interventions to older residents experiencing recent physical health deterioration, with the aim of promoting recovery, independence, and preventing hospital admissions. As a Rehabilitation Assistant Practitioner, you will be responsible for delivering care in patients’ homes or designated community settings, assessing and implementing treatment plans for a delegated caseload using clinical reasoning and analytical skills. In addition, you will manage the ordering and tracking of equipment and supplies, ensuring timely delivery and functionality to support effective patient care. This role offers the opportunity to be part of a committed and caring multidisciplinary team making a tangible difference in the lives of older adults across Hammersmith & Fulham, as well as within the broader community rehabilitation network in Westminster and the Royal Borough of Kensington & Chelsea.

Main duties of the job

As a Band 4 Rehabilitation Assistant Practitioner, you will independently assess a defined caseload of patients using clinical reasoning to identify rehabilitation needs and establish personalised goals, accurately documenting all interventions. You will deliver nursing, physiotherapy, and occupational therapy treatments as part of agreed care plans, working across a range of settings including patients’ homes, and in collaboration with families, carers, and support workers. The role involves providing both individual and group therapy, supporting complex cases under the supervision of qualified professionals, and appropriately escalating concerns beyond your scope of practice. You will be responsible for ordering and fitting mobility aids and equipment, training patients and carers in their use, and supporting the development of therapy students and new staff. Effective communication, including with patients who have neurological impairments, is essential, as is maintaining accurate records in line with Trust standards. You will contribute to multidisciplinary team discussions, manage your own caseload, assist in service development and audits, follow consent procedures, apply outcome measures, and carry out risk assessments to support safe and effective rehabilitation interventions.

Qualifications

Essential criteria

1. Attained at least Level 2 Maths and English (equivalent to GCSE grade 9-4 (A*-C)).
2. Evidence of continuous development.
3. Care certificate
4. Foundation Degree in Health & Social Care/NVQ Level 3 or above or equivalent.

Desirable criteria

5. Recognised qualification in relevant subject.
6. Attendance on Assistants Training Programme Trusted Assessor trained

Experience

Essential criteria

7. Previous experience in NHS, Healthcare or Social Care
8. Working with patients who have multiple diagnosis including frailty, falls, dementias, neurological conditions
9. Able to work in a team
10. Have knowledge and understanding of the role of Rehabilitation professionals in the community
11. Have a knowledge of conditions that can lead to disability and their impact on individuals.
12. Ability to question, clarify, knowledge of own limitations and importance of risk assessment
13. Knowledge of aids and equipment used to promote independence
14. Awareness of confidentiality
15. Clear understanding of role and commitment to role
16. Work in multi-cultural settings
