Team Manager – Transition Team
* Location: City-wide (soon to be North and South based)

* Grade: 6 – Pay Range £54,495 – £64,811
* Hours: 36.5 per week
Responsibilities
We are seeking an experienced and motivated Team Manager to lead a Transition Team, supporting young people with disabilities and additional needs as they move from Children’s Services into Adult Services. The postholder will lead through change and shape future ways of working, providing strong leadership to a team of social workers and practitioners, ensuring high-quality, strengths-based transition planning in line with relevant legislation, statutory guidance, performance, quality, risk and safeguarding oversight, and working closely with partners across Children’s Services, Adult Social Care, Health, Education, providers and families to ensure smooth, person-centred transition pathways.
Qualifications
* Qualified social worker with current Social Work England registration.
* Substantial management and supervisory experience.
* Strong understanding of transition pathways, learning disabilities, autism and complex needs.
* Confidence leading teams through service change.
