Community Nursing Specialist Practice (CNSP) (Internal Only)

This position is open to staff working at the Trust in the 'ECT Family' this includes currentEast Cheshire NHS Trust employees, both NHSP and ECT bank staff, ISS staff and volunteers at the Trust. Unfortunately, we cannot accept applications from applicants not listed within these groups.

We are pleased to be able to offer the opportunity to undertake the District Nurse Apprenticeship at Manchester Metropolitan University (MMU)

This is a level 7 PGDip programme for nurses seeking to qualify in the field of district nursing. Please note holidays are timetabled within the programme with no flexibility.

Entry Requirements:

* Have an NMC-valid registration as a registered adult nurse
* Currently working in Community Nursing
* Recent study at level 6 (evidence required)
* Minimum one year post registration experience at the point of the programme commencing
* Up to date with mandatory training
* Disclosure and Barring Service Check: This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to check for any previous criminal convictions.

Apprentices will be seconded on their current pay band for the duration of the apprenticeship. Upon being shortlisted, candidates will be expected to undertake a numeracy assessment. Candidates will be expected to pass this assessment with a pass rate of 80%.


Main duties of the job

To provide holistic, high quality, safe and skilled health and social care/support to patients, carers and their families.

The programme is designed to prepare the individual to undertake the following duties:

* To have overall responsibility for the assessment, planning, provision and evaluation of District Nursing Care as detailed in the District Nursing Specification.
* To provide effective management and leadership to all members of the nursing team, including recruitment, supervision, appraisal, training and development.
* To contribute to and actively participate in service development based on identified needs and driven by the national and local health care agendas.
* To prescribe effectively as a non‑medical prescriber.
* To be able to work at an advanced level of clinical decision making.

Apprentices will strengthen their leadership attributes; become skilful specialist practitioners; and learn how to lead and bring about change in their practice environments.

* All successful graduates receive a Postgraduate Diploma for Community Nursing Specialist Practitioner Qualification in the field of District Nursing with Integrated Independent and Supplementary Prescribing (V300) at Level 7.
* Graduates also receive 120 level 7 credits and the opportunity to top up to a MSc Specialist Community Nursing (self‑funding route).

Qualifications

* Current NMC (Nursing and Midwifery Council) Registration Registered Nurse (RN) with degree level qualification
* Post registration qualifications e.g. palliative care, care of the elderly, wound care, continence or other relevant qualification.
* Mentor, Preceptor or equivalent


Knowledge & Skills

* Demonstrable experience at band 5 with either community experience or transferable skills.
* Achieved band 5 competencies required for a community post e.g. wound care
* Experience of mentoring junior, newly qualified staff and carers
* Advocate of multidisciplinary team working
* Knowledge and understanding of adult protection issues and safeguarding vulnerable adults and think family.
* Recognition of health & well being agenda including health promotion and admission avoidance.
* Awareness of evidence-based practice and NICE guidance.
* Demonstrable experience at band 6 with either community experience or transferable skills.
* Evidence of leadership skills
* Clinical supervision
* Experience of undertake Continuing Health Care (CHC) assessments
* Advance communication skills.
* Advanced care planning and health coaching.
* Ability to forward plan and meet service needs i.e. appropriate scheduling and management of staffing issues.


Experience

* Rapid decision making and problem solving.
* Ability to lead and manage a team.
* Ability to organise the workload, able to delegate and prioritise.
* Good knowledge of health and safety and risk management.
* Effective written and verbal skills
* Relevant experience in assessing and managing of long‑term conditions, palliative care conditions and wound care.
* Ability to manage time and caseload effectively.
* Provision of support and direction to learner nurses, Health Care Support Workers, Nurse associates and new starters.
