You'll be working within a supportive, close-knit nursing team,alongside experienced nurses, healthcare assistants, and 5 GP Partners. As partof our proactive and compassionate team, youll play a vital role in providinghigh-quality, patient-centred care.
Main duties of the job
In line with the NMC Code of Conduct and our Practiceguidelines, youll:
Deliverassessment, treatment, screening, and health education services
Provideexpert nursing support to patients with Long-Term Conditions (experiencein Diabetes management is essential)
Workcalmly and confidentlysometimes independentlyin a dynamic and friendlyenvironment
AssistGPs and other clinical team members in delivering excellent care

Job responsibilities
Specific Duties:
Topractice unsupervised and independently in accordance with the NMC Code ofConduct and within individual competencies, with reference to Practiceguidelines and referring on to other team members as appropriate
To performroutine tasks related to patient care, to include:
oUndertakingcervical cytology
oInjections andimmunisations (including childhood immunisations)
oProviding acomprehensive travel advice and vaccination service
oObtainingpathology samples, swabs, urine, blood etc.
oInitiate andperform ECGs
oNew patientchecks
To chaperone GPsand assist patients (where appropriate) during clinical procedures
To assist withminor surgical procedures as necessary
To providegeneral and specific health screening and health education to the practicepopulation with referral to GPs as necessary
To advisepatients on general health care and minor ailments
To order andmaintain adequate supplies of stock (including vaccinations) and equipmentrelating to patient care
To be aware ofthe need for regular maintenance of clinical equipment and report potentialproblems with clinical equipment to Senior Nurse Manager
To maintainaccurate documentation of care and utilisation of IT
To undertakeadministrative tasks relevant to patient care and individual CDM clinics
To advise SeniorNurse of potential problems or errors within the range of assigned tasks
To develop thecare of patients with complex health needs, which may involve identificationand assessment of such patients and the subsequent introduction and use oftechnical and specialist nursing treatments
To undertakechronic disease management (CDM) (i.e. asthma, diabetes, hypertension, CVD)where appropriate training has been received. In particular, experience in diabetescare is essential.
To monitor anddevelop individual chronic disease management plans and ensure the smoothrunning of these plans; agreeing these as appropriate with the patient andother health professionals
To prescribeproducts (e.g. dressings, inhalers etc.) appropriately and within expertiseto meet the needs of patients within the practice population
To report allconcerns or issues relating to the organisation of chronic disease managementservices, childhood immunisation clinics, cervical screening programme andhealth promotion services to the Senior Nurse Manager
To be involvedin the development, implementation and evaluation of both new and existingchronic disease management clinics; childhood immunisation clinics; cervicalscreening programme and health promotion services as required

Qualifications
* Degree in Nursing
* NMC Professional body membership
* Basic literacy and numeracy skills
* Basic computer skills essential
* Ability to communicate and deal sensitively with patients and colleagues, both face-to-face and over the telephone
* Friendly and a good sense of humour
* Good organisational and planning skills
* Reliable and able to work effectively as part of team
* Experience of working in a customer care environment desirable
