Job Description American Traveler is seeking a Registered Nurse Case Manager with a DC RN license and at least 2 years of relevant experience for an in-office day shift assignment in Washington, DC.
Responsibilities Work in an office setting with required in-person duties at a wellness center in Washington, DC
Case management for chronic diagnoses including high-risk pregnancy, postpartum, diabetes, CHF, COPD, asthma, obesity, and behavioral health
Full-time day schedule, 5x8 hour shifts Monday through Friday from 8:00 AM to 4:30 PM
No weekend or on-call requirements
Responsibilities include comprehensive assessments, care plan development, and ongoing patient management using GuidingCare/Healthedge EMR
Business casual dress code applies
Duties involve making calls to enrollees, performing face-to-face assessments, and managing care plans
Modules and training will be provided prior to and during orientation
First-time travelers with a strong background are welcome to apply
No time off (RTO) allowed during the first two weeks of the assignment
Must not have been employed by MedStar within the past year to be eligible
May come into contact with COVID-19 patients
Applicants living within 60 miles of the assignment are considered local
Requirements Active DC RN license required by start date; pending licenses accepted with proof of application
Current BLS certification required
Minimum 2 years of experience in acute hospital, long-term acute care, rehab, skilled nursing, care coordination, discharge planning, and admission criteria
Experience with InterQual and Milliman guidelines required
Knowledge of CPT codes, Department of Health DRG, HEDIS measures, NCQA, OSHA procedures and JCC required
Previous charge nurse experience preferred
Must provide two supervisor references for consideration
