To support the Unit Manager and Registered Manager to monitor, support and supervise Reablement Care & Support Assistants in the delivery of quality services. Acting as a point of contact supporting staff, service users and other stakeholders in the absence of the Unit Manager. ● To support the Unit Manager in the day to day running of the service, taking line management responsibility for Reablement Care & Support Assistants on shift. ● To provide all aspects of personal care, support and assistance identified as key outcomes for the service user and contained in their individual service delivery plan or care plan. ● To support service users with medication requirements in line with company policies, if required as part of the individual's care plan. ● To ensure that Reablement Care & Support Assistants maintain care/support of high quality and as directed by individuals care plans. ● To ensure in conjunction with Unit Manager & Registered Manager that the Reablement Care & Support Workers deliver services in line with approved policies and procedures. ● To provide support and guidance to Reablement Care & Support Workers on day to day issues relating to service delivery. To resolve service delivery problems as identified. ● To complete risk assessments in line with agreed policies and procedures ● To report to Unit Manager/s any issues of concern in the care/support of service users, eg. POVA issues, health and safety issues, and general wellbeing issues. ● To review, monitor and support new Reablement Care & Support Workers during their induction period as directed by the Unit Manager. ● To comply with legal requirements: (Care Standards Act 2000 and National Minimum Standards; Health and Safety at Work Act, etc) ● To maintain effective liaison with other personnel involved in the care/support of the Service User, i.e. Community Nurses, Social Workers, G.P’s, etc., as part of a caring team. ● To work flexibly according to the needs of the service and work in a rota system to ensure there is an adequate number of Senior Reablement Care & Support Workers, seven days per week, 24 hours per day. ● Participate in all training courses and maintain personal and professional development. ● To undertake formal supervision of Reablement Care & Support Workers as identified by the Unit Manager in line with departmental and legislative requirements ● To be responsible for any day to day decisions within the confines of the duties allocated and Job Description. You must demonstrate, and model reablement to your team, the aim is to support our service users to go home following a short reablement stay, we will be working with the NHS teams to achieve this. Essential ● NVQ/QCF Level 2 Care Award or obtained within two years of appointment. ● Minimum 2 year's experience working in a care setting ● Knowledge and understanding of the relevant Health and Safety Legislation for Movement and Handling. ● Understand the effects of ageing, disability, incapacity and illness and the effects these can have on a clients' well being. ● Understand an enabling approach to care delivery * Must have Knowledge and understanding of medications.
