Job Overview
The post holder will be responsible for leading safe, resilient and consistent team management and facilitating safe and effective discharge in line with the Trust policies and procedures, supporting the planning and co‑ordination of patient flow across the care pathways. This will involve working in collaboration with the ward teams, key partner organisations, service users, families, carers and other key agencies to ensure meaningful admission and safe, appropriate discharge. This is a senior leadership role focused on the quality of patient experience through delivery of high quality, safe and effective care and the post holder will demonstrate the Trust values and behaviours.
Main Duties
* Provide specialist advice and support to wards and multi‑professional teams and community services in managing effective patient discharge as an integral part of care management across the Trust.
* Maintain a collaborative working interface with Community and Home Treatment Teams to identify patients suitable for discharge and promote a smooth discharge pathway from inpatient services.
* Work with the integrated flow team and other services to identify practical help that can be provided to clients in achieving a timely discharge.
* Lead and actively role‑model high standards of care as part of an effective member of the multi‑disciplinary flow team.
