Community Support Worker (Neighbourhood Team) – Permanent, Full Time

Salary: £25,395 to £32,131 per annum

We have an exciting opportunity within Early Intervention in South Essex for a Community Support Worker. You’ll be working within the Neighbourhood Team, based in Basildon, covering the East and Central Basildon area. As an ‘Community Based Worker’ the role will involve a combination of remote, community and office based working. There is an expectation to attend the office at Ely House, Basildon, approximately 2-3 days per week in addition to ‘as and when’ there is a business need. The role requires mobility throughout an operational area and you will need a driving licence and access to a vehicle, or the ability to meet mobility requirements through other means.
Responsibilities
As a Community Support Worker, you will:
* Undertake assessments and reviews in line with statutory duties under the Care Act to identify eligible needs.
* Generate a person-centred and outcome-focused support plan.
* Work in partnership and develop relationships with internal and external colleagues/partners to achieve the best outcomes for Adults and Carers.
* Regularly visit residents in their homes, meet colleagues, and work from the office or from home.
* Contribute to a varied role supporting adults of all ages (18+), with a commitment to learning and development. An induction and appropriate training will be provided.
This is a large Adult Social Care service that prioritises resident choice and independence, helping people live full and fulfilling lives in their communities. A brief video overview with insights from ASC team members is available.
About You
* Be curious and think creatively to achieve the best outcomes and engage with the community.
* Ability to build effective relationships with adults and their families in times of challenge and crisis.
* Drive and passion to make a difference and a willingness to learn.
* Ideally some experience of activity-based work with adults and knowledge of legislative and policy guidance applicable within social care settings.
