As a Senior Primary Care Nurse, your responsibilities will vary. You will:
1. Lead a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions.
2. Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening.
3. Provide outstanding evidence-based care to our patients, seeking out, listening to, and acting on their feedback, so that care is personalised and informed by what matters to them.
4. Deliver non-judgemental care that makes a real difference.
5. Inspire excellence by forging strong links with our partners and stakeholders and providing a service that people can trust, feel safe within, and feel proud of.

* Registered nurse Adult with current professional qualification (NMC).
* Evidence of Continued Professional Development.

Hours of work:
* Monday to Sunday service.
* 37.5 hours per week.
* Various shifts between 07:30 - 20:45. This is a full-time position with an annual salary between £42,094 - £47,466 per annum FTE, depending on experience.
