Recovery Coordinator - Reconnect to Health - HMP Durham Location: HMP Durham, Old Elvet, DH1 3HU Working Hours: 37 hours per week Monday to Friday 08:00am-16:00pm Contract Type: Permanent Salary:£25,877.80-£34,240 Please note: The salary listed reflects the full earning potential for this role. About the Role To work as part of a team to provide collaborative recovery planning and case management to substance misusers (drug and alcohol) and their families across all stages of individuals’ recovery journeys, from assessment and engagement through to sustained recovery. You will work collaboratively with colleagues based within prisons and community treatment services, to support individuals both whilst in custody, through to release. What You’ll Do Provide specialist, tailored support to enable service users to make positive changes. Complete comprehensive assessments for individuals who require support from the Drug and Alcohol Recovery Team in HMP Durham. Effectively manage a caseload of service users, reviewing and updating on-going processes such as risk assessments and personalised recovery plans. Work closely and collaboratively within the multi-disciplinary team across the service and externally, through attendance at reviews, to support individuals in their recovery journey. Support the facilitation of psychosocial interventions that meet local need across both group work and one to one formats. The role involves effective use of an online case management system. Promote visible recovery across the service and ensure peer support opportunities are relevant and accessible for all service users. Ensure safeguarding processes are fully adhered to, identifying and preventing potential harm to service users and others by following local and organisational safeguarding guidelines, including referral to and engagement with relevant safeguarding authorities To Succeed in This Role, You’ll Need: A desire to support individuals with substance misuse issues, through their recovery journey. Knowledge of harm reduction approaches in relation to substance misuse. Understanding of and ability to implement interventions, including brief interventions MI, PSIs, CBT and ITEP. Working with models of service delivery and issues relating to recovery planning. Working with best practice frameworks e.g. NICE, DOH, NTA, NMC, Care Quality Commission & Caldicott Standards. Delivering one to one and group therapeutic interventions. Excellent partnership and stakeholder engagement skills. Ability to maintain professional boundaries and resilience whilst supporting individuals intensively, sometimes in stressful situations.
