Job Overview

Respiratory Nurse Specialist, Band hours a week)

The Respiratory Nurse Specialist will work as a member of the Integrated Respiratory Team in Gloucestershire, within the acute setting to support both Inpatient and Outpatient Services for a variety of conditions such as Asthma, COPD, Bronchiectasis, ILD, oxygen assessments and providing follow-up care through the virtual supported discharge service. This is an exciting opportunity for an experienced nurse to step up into a specialist nursing role. The post holder must be willing to work weekends when required.

Within the role the post holder will be an integral part of the Respiratory Clinical Nurse Specialist team ensuring robust communication with members of the Respiratory team, MDT, GPs, patients and relatives. They will also be responsible for seeing patients in the hospital assessing them for oxygen, completing COPD or asthma reviews including bundle and audit and assessing patients for the virtual ward. They will then provide virtual consultations to the patients on the virtual ward each day. The post holder will also support or lead a sub speciality to ensure expert knowledge and service development.

Detailed Job Description And Main Responsibilities

To provide supported discharge for respiratory patients and facilitate early discharge from the wards
To support admission prevention by providing specialist support through in reach to unscheduled care.
To act as an accessible specialist resource and educator within the Gloucestershire Respiratory Service, providing education and training to colleagues within the trust, and members of primary and community care teams.
Proactively participate in quality and service improvement, working closely with the Lead Respiratory Nurse, Senior Matron and multidisciplinary team.
Facilitate and support discharge of patients from hospital to home
Follow up patients post discharge to prevent inappropriate admission to hospital
Participate in the implementation of local policies and procedures to improve the effectiveness of the department
To participate in the day to day co-ordination of the virtual supported discharge service
Respond effectively to referrals within stated timescales
Coach patients and carers on how to access help when they feel that their condition or needs have changed, including what to do out of hours
Be responsible for data collection and inputting to databases for local and national clinical audits
To be involved in monitoring and evaluation of patient feedback, offering recommendations as part of the team to improve services
Maintain accurate patient and staff records in line with Trust guidelines.
Support the MDT to facilitate safe and effective transition of care between hospital and community services in order to provide seamless support for people.
Respond to queries from patients, carers and visitors providing verbal and written information in a timely manner
Arrange delivery of consumables to patients when required.
Maintain confidentiality at all times in line with Trust and Data Protection policies
To be able to prioritise your workload and manage your own time and caseload effectively
Good organisational skills and the ability to prioritise effectively
To participate in negotiating individual and highly complex care programmes and liaise with other members of the multidisciplinary /multi agency team in the planning, delivering and evaluating care to improve outcomes for service users.
To manage and record complex clinical and management information on the appropriate IT Systems/documentation in accordance the policy guidelines and data protection act.
To identify, apply and disseminate relevant research findings within the area of practice to support clinical effectiveness and the development of the service.
To participate in the teaching, assessing and support of student nurses and NVQ students within their community placements, meeting the aims and objectives of the university/college where needed liaising with tutors.
To provide specialist advice, support and care to patients and their relatives, facilitating high quality, evidence-based care in collaboration with the multidisciplinary team and Specialist Centres.
Contribute to the Trust Clinical Governance Agenda through education, clinical guideline production, audit and research within the respiratory field.

Registered Nurse or AHP
Degree Qualified or equivalent level of study or experience
Independent prescriber or willingness to undertake

Additional relevant qualification at diploma/degree ideally in respiratory
Teaching/Assessing Qualification

Experience

Essential criteria

Extensive experience of working in the NHS in the community or hospital setting
Appropriate experience of working in a respiratory care environment
Experience of managing long term respiratory conditions such as COPD, asthma, ILD, Bronchiectasis

Desirable criteria

Experience in managing patients during an exacerbation of a long term respiratory condition
Experience of working in partnership with community services

Knowledge / Skills

Essential criteria

Excellent computer skills, a thorough understanding of Microsoft office applications including Excel and will be able to manage audit and National databases
Excellent communication (written and verbal) and listening skills.
