Change Grow Live in Burnley is seeking a Young Adult Transition Worker to help young adults transition from young people's services to adult services. This important role provides the opportunity to engage with young adults affected by drugs and alcohol and support their path towards recovery.
As a part-time role with 30 hours per week, you will take on a caseload and work in a dynamic team environment, ensuring a service user-focused approach and leading by example.
