Workingas part of Marshalls PCN to support the Clinical Pharmacist team to help on thedelivery of safe, effective and efficient systems for repeat prescribing,medicines optimisation, reducing medicines waste and maximising patientoutcomes. The role will also include training and development to support thewider MDT team improving co-ordination and effective pharmacy administrationwithin general practice.
Main duties of the job
* Undertake medicines reconciliation in accordance with practice protocols and to update patients medical records - prescribing accordingly.
* Support the achievement of the prescribing Quality & Outcomes Framework targets and other clinical targets
* Provide advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness.
* Participate in medication audits.
* Assist in the appropriate monitoring and management of prescribing budgets and to liaise with the medicines management team in the local ICB.
* Aid in any Quality Improvement (QI) projects that are appropriate.
* Respond to medication queries from patients and staff in a professional manner referring to the appropriate clinician in accordance with practice protocols.
* Initiating contact with and responding to requests from patients, other team members and associated healthcare professionals and providers.
Assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits.
* Be responsible for the organisation, planning and of own workload to meet set deadlines.
* Taking messages and passing on information as appropriate.
* Signposting patients to the relevant community services
* Computer data entry/data allocation and collation, processing and recording information in accordance with practice procedures.
* Any other duties that may help the PCN and PCN Pharmacy team in their workload.

meets the specific qualification and training requirements as specified by the GPhC criteria 74 to Register as a Pharmacy Technician.
Enrolled in or willing to enrol in an approved training pathway. Ie. the Primary Care Pharmacy Educational Pathway (PCPEP)
Is working under appropriate clinical supervision to ensure safe, effective and efficient use of medicines
Health and Safety - Risk Management
Must always comply with the Health and Safety policies, in particular following safe working procedures and reporting incidents using the organisations Incident Reporting Systems
Comply with the Data Protection Act (2018) and the Access to Health Records Act (1990).
Special working conditions
The post holder is required to travel independently between work sites and to attend meetings etc hosted by other agencies.
The post-holder will have contact with body fluids, i.e. wound exudates, urine etc while in clinical practice.

Qualifications
* Registered with the GPhC as a Pharmacy Technician Evidence of CPD
* Member of the Association of Pharmacy Technicians (APTUK)
Experience
* Experience of prescribing / clinical systems such as SystmOne, EMIS Web
* Recent previous experience within comparable role in General Practice
* Previous experience working within a patient facing role
