Eliot’s Respite program is a short term restorative service that assists clients with obtaining and maintaining community living. As a Case Manager, you will be responsible for providing This person will provide case management and coordination for 8-12 consumers. Our ideal candidate will have the skills to establish supportive relationships, excellent communication, crisis management and problem solving skills. Responsibilities: Provide intensive and mobile case management, care coordination and support and interventions to assigned consumers either in the community or at site-based Respite. Coordinate and case manage the day-to-day needs and overall service delivery of consumers at Respite or in the community as assigned by the Clinical Supervisor or Program Director. Support and assist consumers with day-to-day needs and overall service delivery providing direct support and community linkage to meet identified needs. Coordinate and facilitate services which may include assisting consumers with managing income, insurance, other entitlements, accessing other services and community resources, and scheduling/coordination of all medical and psychiatric appointments. Develop IAP’s (Individualized Action Plans) for each consumer as assigned. Ensure that treatment goals are culturally competent, strength-based, realistic, attainable, and promote recovery. Provide intensive supportive casework for assigned caseload, including advocacy and assistance with access to entitlements to social, medical, psychiatric and community services.
