Lead Clinical Pharmacist role at Alfianni Online - Artist Netwerk. Band 8a salary: £55,690 – £62,682 pro rata.
Responsibilities
* Organise and develop clinical pharmacy services within designated patient populations. Classification may include a primary care network, Directorate service, or other population/cohort across Newcastle geography, e.g. care home residents or the frail population. Build and develop relationships between local community pharmacies and other NHS services.
* Promote the safe, rational and cost effective use of medicines by working with colleagues from all disciplines to share good practice and influence prescribing behaviours where necessary.
* Perform the professional role of a registered pharmacist, including advice, supervision and cooperation with colleagues. Act as a registered Independent Prescriber (Non-Medical Prescriber) within the scope of competence, including prescribing and de-prescribing.
* Provide expertise in structured clinical medicines review and address public health and social needs of patients across settings.
* Deliver services across settings (secondary care, primary care, intermediate care or domiciliary) as required to meet patient needs. Run clinics where medicines are central to managing long-term conditions (e.g., titration of heart failure and hypertension medicines, cardiovascular risk reduction, anticoagulation, chronic lung disease management).
* Help patients achieve health goals, reduce inappropriate polypharmacy and medicines waste through structured clinical medication review. Reconcile medicines after transfers between care settings and coordinate with patients and usual care providers to ensure continued access post-transfer.
* Contribute to reductions in medicine-related hospital admissions and readmissions by addressing medicines-related issues and supporting optimal outcomes. Collaborate with community, medicines optimisation and hospital pharmacy colleagues; develop referral processes between primary care professionals and promote Stock Shortage Protocols and Repeat Dispensing and New Medicines services.
* Support the development of safe and efficient prescribing and medicines management systems within the designated population. Advise on prescribing workflow, share best practices across care settings, and promote facilitative technologies.
* Manage medicine queries from patients and health care professionals. Implement drug withdrawals, shortages and alerts from bodies such as the CCG or MHRA to improve medicines safety; work with colleagues to implement NICE, Shared Care and other evidence-based guidelines.
* Contribute to multi-morbidity reviews and multidisciplinary reviews. Provide medicines information, clinical supervision and training to healthcare professionals and administrative staff. Review pathology results and act as a source of medicines information for professionals, patients and carers (e.g., dosing, side effects, adverse events, alternatives).

* Undertake up-titration of Heart Failure and Hypertension medicines, minor ailments triage, and support walk-in centre activity where required. Reconcile medicines after transfers from acute care and promote post-transfer access.
* Manage repeat prescribing reauthorisation, and respond to medicine queries. Implement drug withdrawals and alerts with the aim of improving medicines safety; collaborate with primary care professionals to implement NICE guidelines.
