Service Care Solutions are currently recruiting for a Community Psychiatric Nurse to join a Dementia Service within the NHS, based in Telford.
Contract Length: Initially 3 months, with potential for extension
Hours: Full-time, Monday to Friday, 9am–5pm
Location: Community-based roleKey Responsibilities:
Provide specialist mental health support to individuals with dementia in the community
Conduct assessments and develop person-centred care plans
Administer prescribed treatments and monitor patient progress
Liaise with families, carers, and multidisciplinary teams to ensure continuity of care
Contribute to safeguarding processes and attend relevant meetingsRequirements:
Previous experience in dementia care and community psychiatric nursing
NMC registration
