Overview

Care Co-ordinators are key players in the PCN vision to ensure that patients have access to a wide variety of care options alongside the excellent primary care provided by GP practices. This role involves managing a caseload of patients, liaising with patients and families to help them access the support they require, as well as running computer searches and proactively contacting patients to discuss health and care issues. The post holder will work with Havant and Waterlooville PCN practices and colleagues across secondary care, community health, adult social care and the voluntary sector. Interviews will be held on Wednesday 15 October 2025. Successful applicant will work 25 hours per week.


Main duties

* Provide co-ordination and navigation for people and their carers across health and care services, working closely with Ageing Well Social Prescriber, Ageing Well Frailty Nurses and other primary care professionals.
* Manage own workload to ensure the needs of individuals who use our services are met, taking the wider needs of the team into consideration.
* Refer people onto other health professionals where required and appropriate.
* Provide personalised support to individuals to help them be active participants in their own care solutions.
* Help people manage their needs by answering queries and ensuring that patients have good quality information to help them make choices about their care.
* Support personal choice while ensuring individuals understand the accountability of their own actions and decisions.
* Work in partnership with the wider Ageing Well Team to connect patients to community-based care solutions and services.
* Support the co-ordination and delivery of MDTs within the Ageing Well Team and other community providers.
* Communicate information in a way that is relevant and understandable for service users and carers, in line with Practice/PCN standards and policies.
* Ensure safeguarding processes are in place for vulnerable individuals.


About Us

Havant and Waterlooville Primary Care Network (PCN) has five member practices in the south of Hampshire, serving around 67,000 patients. Primary Care Networks were established in 2019 to extend staff roles and services available from GP surgeries by working across neighbouring practices.

Job Description

Job responsibilities

Patient-facing

* Work closely with the Ageing Well Team and other primary and secondary care professionals to support patient care.
* Undertake person-centred assessments for patients allocated to you and develop individual health and care plans; consult with appropriate colleagues as required.
* Ensure care plan reviews are held within the required time frames.
* Provide personalised support to individuals/families to empower them to be active participants in their care.
* Answer queries and provide written or verbal information to help patients make choices about their care; support personal choice while ensuring accountability.
* Ensure interactions adhere to the principles of personalised care and align with standards and policies.
* Support patients to build knowledge, skills and confidence and connect them to relevant community-based care solutions or other services.
* Participate in risk assessments and determine appropriate actions, consulting with colleagues as needed.
* Ensure safeguarding actions are taken for adults and children as necessary.

Analytical and Judgement

Be aware of personal competencies and practise within them; review information promptly and progress PCN outcomes; escalate safeguarding concerns as needed.

Treat all patient and staff information as strictly confidential in accordance with policy.

Health and Safety

Promote health and safety as defined in policy; identify risks, undertake training, and maintain clean and safe work areas.

Personal and Professional Development

Participate in training and development, including induction, annual reviews, shared decision-making modules, CPR training, and mandatory training.

Communication

Communicate effectively with the team, patients and other agencies; adapt communication methods as needed.

Duties will vary with workload and capacity and may be updated by management as required.


Person Specification

Experience

Essential

o Minimum 2 years of experience working with healthcare professionals and/or NHS or social care or related field.

Desirable

o Experience of working in a multi-disciplinary setting.

Qualifications

Essential

o Educated to A-Level / NVQ level 3 or equivalent; willing to undertake relevant training.

Desirable

o Diploma level BTEC/HNC/HND or equivalent;
