About the Role:
Your responsibilities as a Support Practitioner may include:
* Social Support: Assisting individuals to engage with their communities through outings and activities such as day centres, bowling, cinema, swimming, etc.
* Medical Support: Administering and monitoring medication.
* Personal Care: Supporting personal hygiene, dressing, arranging healthcare visits, and assisting with incontinence care.
* Practical Support: Assisting with household tasks like cleaning, laundry, ironing, cooking, and shopping.
