Bank Senior Clinical Practitioner - Oldham

gtd healthcare is one of the largest NHS commissioned Primary Care, Out of Hours & Urgent Care providers in the North West of England.

We have a fantastic opportunity for a Senior Clinical Practitioner (Podiatrist or Nurse) to provide clinical assessment of patients with peripheral arterial and venous disease within a community-based meeting of Multidisciplinary Team (MDT) Vascular clinic across the Oldham area on a 12 months fixed term contract.

The Well Leg service will consist of patients who have been referred by community or secondary care clinicians by suspected peripheral arterial disease (PAD) or venous disease. The patient journey will require a comprehensive vascular assessment, diagnosis (including ABPI and TBPI) and a management plan, which may include management of modifiable risk factors, medication review/recommendations, lifestyle advice / health promotion, compression hosiery prescription and onward referrals where appropriate.

The successful candidate will be an experienced clinician (Podiatrist/ Nurse) with expertise in comprehensive arterial assessment including ABPI and TBPI, knowledge of venous disease and ability to assess for compression hosiery. You will have a genuine passion for lower limb assessment and management and a commitment to delivering excellent patient-centred care within a primary care setting.

Variety of shifts available, Monday to Friday between 8:30 and 17:00.


Main duties of the job

* Supporting with training and undertaking of comprehensive assessments, diagnosis and management of patients with PAD and venous disease.
* Providing holistic care to patients with vascular disease, including monitoring, education, and support.
* Leading, supporting and training junior staff with the MDT clinics and innovative ways in meeting individual patient needs (focussing on those that are underserved).
* Collaborating with GPs, multidisciplinary teams and local services to optimise patient outcomes.
* Promoting best practice in line within the current guidelines.
* Providing data and analysis on specific patient outcomes and key performance indicators.

Job responsibilities


Clinical Practice

* Assess, diagnose, plan, implement and evaluate treatment / interventions and care for patients presenting with undifferentiated diagnosis.
* Clinically examine and assess patient needs from a physiological, psychological and social perspective, and plan clinical care accordingly.
* Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
* To demonstrate a level of clinical practice that reflects agreed policies, protocols and guidelines within the clinical strategy framework.
* Work flexibly to support clinical services within scope of practice as per service need.
* Diagnose and manage acute episodes of care integrating both drug- and non-drug-based treatment methods into a management plan.
* Demonstrate senior clinical decision making and critical nursing skills.
* Provide self management advice and education.
* Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
* Support patients and their families to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care where appropriate.
* Manage own workload.


Communication

* Utilise and demonstrate sensitive communication styles, to ensure patients and or their families are fully informed and consent to treatment.
* Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
* Anticipate barriers to communication and take action to improve communication.
* Maintain effective communication within the organisation and with external stakeholders.
* Ensure awareness of sources of support and guidance (e.g. PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

* Recognise and work within own competence and professional code of conduct as per regulation.
* Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
* Demonstrate senior history taking skills appropriate to clinical setting.

* Deliver care according to NICE guidelines and evidence-based care.
* Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
* Ensure professional revalidation requirements are met.
* Participate in the maintenance of quality governance systems and processes across the organisation and its activities.
* Utilise the audit cycle as a means of evaluating the quality of the work and implementing improvements where required.
* In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.
* Work with senior staff in identifying any areas of improvement in response to patient feedback relating to health care provision and the effectiveness of care.
* Support and participate in shared learning across the organisation and wider organisation.
* Work with the Advanced Practitioners to assess the impact of policy implementation on care delivery.
* Understand and apply guidance that supports the identification of vulnerable and abused children and adults and be aware of statutory child/vulnerable patients health procedures and local guidance.


Non-Medical-Prescribing

* If you are employed in a prescribing capacity, you must only do this if qualified and competent to do so. This applies to those practitioners with V300 qualification, who have active registration with the NMC or the practitioners professional regulatory body.
* You must maintain your knowledge and skills with regards to prescribing and administration of drugs, keeping up to date with any changes, alerts, recalls and other reasonable professional knowledge.

* Contribute to the professional development of the clinical workforce in collaboration with the Chief Nurse, Head of Clinical Services and Clinical Leads.
* Participate in staff development to maximise individual potential, provide clinical supervision, always acting as a positive role model.

Person Specification


Experience

* Experience of leading/managing quality improvement.
* clinical examination skills.
* Working in Primary Care/Urgent/Emergency Care.
* Clinical supervision/assessor training and experience.


Qualifications

* Registered RGN/HCP/GPhd/RPS.
* Registered with appropriate regulating body.
* Recognised clinical exam skills course and relevant senior work experience.
* Ideally possess a degree qualification.
* Specialist Emergency Care Qualification.
* Non Medical Prescriber.
