Job Summary:
Support Workers are central to Praxis Care’s mission of providing quality care to individuals. Each Support Worker is part of a dedicated team that delivers high-quality, reliable support services.

We can consider a range of different working patterns, including full time, part time and relief.
Key Accountabilities:
* Empower supported individuals to live as fully and independently as possible within their local community, by supporting them to develop life management skills and become independent in the areas of budgeting, cooking, shopping and maintaining their dwelling
* Provide practical and emotional support with mental well-being / addictions / and a range of complex needs
* Provide housing related support with care for the individuals we support, to maintain the security and safety of their dwelling
* Provide housing related support with care which will allow the people we support, to manage their health and wellbeing e.g. support to access advice and information about general health as part of a package of support and signposting to other service
* Empower the supported individuals to access local services and encourage active participation in the community to promote a better quality of life

Requirements of the Role:
* Participate in the company appraisal and staff review process.
* Adhere to and model the company values, behaviours, and competencies at all times.
* Carry out all duties of the post in accordance with Praxis expectations, regulations, and policies.
* Be prepared to take on additional duties and responsibilities as required.
* Work the necessary hours to meet the needs of the service.
* Where the role requires sleep-ins, a minimum additional payment of £45.00 per night applies
The above-mentioned duties are not exhaustive, and the post holder may be required to carry out other duties as necessary. It should be noted that stated duties or locations associated with the post may change to meet the future needs of the organisation.

* Numeracy and literacy skills.

* Experience working within the health and social care field.- desirable

* Full valid driving license and access to own vehicle (If a disability precludes an individual from holding a driver’s license, they should state how they can meet the mobility requirement of the post).
