Advanced Clinical Pharmacist - Antimicrobial Stewardship and Infectious Diseases.
There is now an exciting opportunity to join the antimicrobial stewardship and Infectious diseases (AMS-ID) pharmacy service as an Advanced Clinical Pharmacist. We are looking for an enthusiastic individual to join the AMS-ID pharmacy team and work with the Principal AMS-ID Pharmacist to develop the service over the coming years. You will provide an advanced clinical service to the AMS-ID wards, as well as assisting the Principal AMS-ID Pharmacist to provide leadership within the directorate. You will lead on trust and pharmacy based education and training matters associated with AMS-ID, as well as on AMS-ID service evaluations and audit reporting. The department supports development of electronic solutions and staff development schemes such as the RPS Advanced Portfolio.

Responsibilities
* Liaise with senior pharmacy, medical and nursing staff, other allied health professionals (AHPs) and managers to promote safe, evidence-based and cost-effective use of antimicrobials.
* Provide an advanced clinical pharmacy service to Microbiology and the Infectious Diseases team.
* Provide specialist pharmacist advice and service to designated wards/clinical areas (AMS/ID).
* Act as the nominated pharmacy lead for the development and delivery of training in AMS for pharmacy staff and other staff groups as required.
* Act as the nominated pharmacy lead for AMS clinical audits, service evaluations and quality improvement projects.
* Prescribe medications for individual patients within the agreed scope of practice and CUH prescribing policy.
* Participate in and lead on novel pharmacy projects in the care of patients in designated clinical areas (AMS/ID).
* Identify and undertake research to inform practice developments and underpin delivery of an evidence-based service.
* Provide pharmacy leadership to the designated clinical area (AMS/ID).
* Work towards independent prescribing qualification if not already held.
* Develop and refine models of care through pharmacist-led ward rounds and adapt practices to evolving service requirements.
Qualifications and experience
Essential
* Masters level degree in pharmacy.
* Registered as a pharmacist with General Pharmaceutical Council (GPhC).
* Postgraduate Diploma in Clinical Pharmacy/General Pharmacy Practice (or equivalent).
Desirable
* MSc in Clinical Pharmacy (or equivalent).
* Management qualification.
* Registered as an independent prescriber with the GPhC.
* Member of Royal Pharmaceutical Society.
* Member of the faculty of the Royal Pharmaceutical Society (or working towards) at Advanced Stage II.
Experience
Essential
* Recent significant post-registration experience including prescription screening, medicines reconciliation and medicines information.
* Experience of patient consultations and working as an experienced clinical pharmacist at band 7 or band 8a across multiple specialties including AMS/ID.
* Experience documenting pharmaceutical care issues in medical notes and writing professional reports, drug policies and treatment guidelines.
* Experience developing and reviewing patient group directions and shared care guidelines; conducting and implementing clinical audits; practical application of antimicrobial stewardship; management of high-cost/high-risk medicines.
* Experience as a personal tutor and in leading projects including change management and service development; education and training including supervision and appraisal of staff and students.
Desirable
* Post-registration hospital experience across multiple sites; independent prescribing for a designated patient group; specialty experience (e.g., surgery, neurosciences, critical care, antimicrobial stewardship, transplant, medicines information, acute and emergency medicine).
* Experience writing pharmaceutical/educational publications; drug expenditure analysis and horizon scanning; staff management.
Knowledge
Essential
* Advanced clinical knowledge of drugs and therapeutic trends in the designated area; knowledge of NICE/HEE/NPSA guidance and local implementation; understanding of UK healthcare/NHS changes.
* Computer literacy (Word, PowerPoint, Excel); familiarity with teaching aids; knowledge of pharmacy and electronic prescribing IT systems.

* Demonstrate expert professional practice (RPS Advanced Pharmacy Framework: Advanced Stage II).
* Strong organisation, prioritisation, time management; self-motivated and enthusiastic.
* Excellent written and verbal communication; ability to present to varied audiences; ability to produce written reports.
* Proven ability to influence and negotiate; team player; able to listen to and act on senior direction.
* Ability to review pharmaceutical care within a reasonable time frame and resolve issues; empathy for patients and carers; ability to plan, design and deliver training programs.
