Job overview Nottingham City GP Alliance is seeking a skilled and dedicated Nurse Practitioner (NP) to join our dynamic GP surgery teams across the Bilborough locality in Nottingham City. The successful candidate will provide high-quality care to patients across a range of conditions, working autonomously and collaboratively with GPs and the wider practice team. Advert See Job Description for full details. In summary: 1. Clinical Practice Undertake advanced clinical assessment. Independently diagnose, treat and manage patients Provide same-day and pre-bookable appointments for minor illness, acute presentations and exacerbations of long-term conditions Prescribe safely and effectively as an Independent Nurse Prescriber, Request, interpret and act upon investigations Recognise and appropriately escalate patients whose needs exceed NP scope to a GP or relevant specialist. Maintain accurate, contemporaneous and high-quality clinical records using the practice clinical system. 2. Nursing Leadership & Professional Practice Act as a senior nursing clinician, role-modelling best practice and professional standards. 3. Quality, Governance & Safety Work within clinical governance frameworks to support safe, effective and high-quality care 4. Education & Continuous Professional Development Maintain professional registration and revalidation with the NMC Engage in regular clinical supervision (either peer review or with members of the medical team) and reflective practice. 5. Communication & Patient-Centred Care Communicate effectively with patients, carers and colleagues, including discussion of diagnoses, treatment options and self-care. Working for our organisation Nottingham City GP Alliance (NCGPA) is a federation owned by city general practices whose mission is to support practices to: provide high quality, person-centred, care Address deprivation and acknowledging diversity in our population Develop an effective and sustainable workforce for now and the futur Our vision aims to: Support Nottingham owned General Practice to provide the best level of care to our urban population including our students and those who are disadvantaged Lever funds to strengthen General Practice in the city by having a focus on NHS strategic priorities including inequalities, severe and multiple disadvantage (SMD), digitalisation and recovery from Covid Deliver city-wide services that makes sure people can access the right care at a time and place that is best for them Create a city-wide presence in city wide partnerships Develop a range of services to support practices maximise efficiency in practice administration Facilitate innovative ways for General Practice to enhance its service offer including the use of ARRS roles to create PCN multi-disciplinary teams Develop workforce training pathways for GPs, nurses and other professions in order to improve the working life of people working in the city Detailed job description and main responsibilities Parliament Street Medical Centre is a city centre practice with a growing list size. It has a young population with a low rate of home visits. The practice has a history of achieving exemplary patient care (outstanding CQC rating). It has a high level of patient need around Severe Multiple Disadvantage and the existing team will work to support and develop new clinicians around this. Bilborough Medical Centre & Grange Farm Medical Centre are in the West of the City, serving a very deprived area of Nottingham. They have a more traditional population with significant health problems and complex needs. The practices are looking to develop and embed innovative multidisciplinary team working and intend to expand this further to include many of the new ARRS roles. Grange Farm provides the lead care home role for City South PCN. Greendale Primary Care Centre serves patients across Sneinton and St Annes in the East of the City. Also working in an area of significant deprivation with a significant number of patients where English is not their first language, Greendale PCC is an established GP training practice, and the training leads are supporting NCGPA to develop training across our other sites. Each of our sites have GP and management leads to support the team and they in turn are supported by the NCGPA Medical Director and Head of Practice. NCGPA has a robust clinical and operational governance process to support quality and safety across the organisation. NCGPA is developing a new model of collaboration across our sites, to standardise systems, share and expand back-office functions. Our ambition is to create a new model of resilient care for the city, preserving and improving primary care services for the citizens of Nottingham.
