Calling all Physical Health Nurses, we have a rare opportunity of a 6 month secondment into the Falls Prevention Service as a Falls Specialist. The Falls Prevention Service is a small team within the Primary, Community and Dental services directorate undertaking comprehensive geriatric assessments in both clinics and in patients homes across Oxfordshire.

A Falls Specialist sees people who have experienced falls, examining their falls risks, in line with the NICE guidelines, and then formulating a plan to address these risks. Much of the time you will work independently but the team meet up face to face on a Thursday for an MDT. As part of your assessment you will perform a variety of tests with the patient including ECG taking and phlebotomy procedures. You must be skilled and competent in phlebotomy.

This is a 6 month fixed term contract / secondment for 30 hours per week.


Main duties of the job

The Falls Specialist's work involves

* Completing a Comprehensive Geriatric Assessment (CGA) in line with NICE guidelines, drawing the information together to conclude what may be causing the person to fall over.
* Providing the patient with a personalised Falls Management plan.
* Sending a report to the patient's GP of your findings and recommendations.
* Advising or completing appropriate onward referrals for the patient.
* Attending a weekly falls MDT to discuss the patients you have seen.

Person Specification


Knowledge

* Knowledge of elderly care
* Knowledge of community work
* IT literate with Word, Excel, EMIS, Electronic Patient Records
* have excellent communication skills verbal and written
* have good knowledge of the wider multi-disciplinary team in the community
* awareness of national and local issues for this client group


Qualifications

* professional qualification degree or equivalent
* comprehensive geriatric assessment or frailty or advance history taking module level 7
* teaching and assessing qualification


aptitude and skills

* able to plan and organise own work
* awareness of stress and anxieties in self and colleagues, patients and families
* working in a multi-disciplinary way


Experience

* experience of working in the community
* work without direct supervision
* ability to carry out clinical assessment
* supervision of other staff or students
* able to work on own initiative
* team player
* able to produce results within a specified time


contractual requirements

* able to travel countywide
* able to carry equipment
* motorvehicle licence with access to a vehicle that can be used for work daily
