Come and join our fantastic District Nursing team in Frome, East Mendip, Somerset.
We have an exciting opportunity for a fixed 1-year contract for a newly qualified or experienced Nurse to work as part of our friendly and dynamic multi-disciplinary care team. You must be an enthusiastic individual with excellent communication skills and motivated in your approach to care delivery to ensure we maintain a high-quality service for our patients.
You will be supported to manage a caseload and work as part of a larger multi-disciplinary, multi-agency team, liaising closely with our Primary Care Networks and Neighbourhood Teams as part of the out-of-hospital model of care.
The community nursing service is commissioned to provide services between 8am and 10pm, seven days a week, including evenings, weekends, and bank holidays. Applicants will be required to work a shift pattern within these hours, although flexibility can be discussed. A driving license and access to a car are essential.
Main duties of the job
Community Nurses work as part of a team providing high-quality and compassionate care to individuals. The Community Nurse is an autonomous practitioner responsible for holistic assessment of both complex and simple care needs, the development, implementation, and evaluation of care, and delegation of care where appropriate to other team members. They will support the multidisciplinary team in delivering high-quality care to enable individuals to achieve their person-centered goals.
The Community Nurse will take responsibility for the patient's care pathway and non-clinical duties in the absence of the Team Leader. They should have the understanding to address complex problems in their daily work and report concerns to a Team Leader.
Community Nurses will work effectively as part of a team and act within their competence limits, knowing when to ask for help. They will operate within workplace standards, policies, and protocols, including the NMC Code, and professional standards of practice and behavior for nurses and midwives.
