An exciting opportunity awaits for an Occupational Therapist to join STSS (Short Term Support Services) in Lancashire. STSS is committed to providing a single point of access to community intermediate care services, collaborating with referring professionals to advance initial assessments and deliver relevant services to individuals in need.
The role involves conducting holistic face-to-face assessments, reviews, and support planning, facilitating the transition onto appropriate services, all outside of the hospital setting. Utilising a strengths-based approach, you will ensure individuals receive timely support tailored to their needs, empowering them to achieve independence and lead a fulfilling life on their terms. Key Responsibilities:

* Collaborate with Community Health & Social Care partners.

Requirements:

* Proficiency in managing risk and promoting positive risk-taking.
* Experience in managing a varied caseload, including complex cases.
* Knowledge of equipment, moving and handling techniques, and single-handed care approaches.
