Overview

Join our Older People’s Mental Health Liaison Service, a friendly multi‑disciplinary team supporting patients across Bradford Royal Infirmary, Airedale General Hospital and community sites. This role offers the opportunity to work across A&E, acute wards and community settings, supporting older people with complex mental health needs. You will lead on assessment, care planning and interventions for patients in general hospital environments and those transitioning between hospital, DAU and care homes, providing expert advice to ward teams and care home staff to ensure high‑quality, person‑centred care and appropriate onward referrals.

Working closely with Consultant Psychiatrists, Community Mental Health Teams and inpatient services, you will play a key role in delivering coordinated care, contributing to clinical excellence, service development and staff training within acute settings.

We are looking for a motivated, flexible practitioner (RMN, OT or Social Worker) who can work both independently and as part of a wider team, make sound clinical decisions and build strong working relationships across services.


Main Duties

* Act as a resource clinician offering support and clinical advice.
* Ensure implementation of all Trust Policies and Procedures as required.
* Receive and accept referrals for the liaison service and initiate, lead and coordinate assessments, making referrals to other professionals as required.
* Act as a resource within acute hospital Trust and care home settings in matters relating to the care and treatment of older people presenting with mental health problems.
* Work cooperatively and actively with clients and carers, encouraging participation in care planning and delivery.
* Contribute to the planning, monitoring and control of resources.
* Ensure agreed standards of care are achieved.
* Apply the Mental Health Act (1983) – maintain knowledge of and act in accordance with Directorate policies and procedures.
* Ensure equality of opportunity and fair access for all service users and carers using the service.
* Take a lead role to ensure that issues of clinical governance, research, audit and good practice are addressed within the service.
* Develop an effective communication network through liaison and collaboration with other disciplines, services and agencies to ensure continuity of patient care.
* Ensure care delivery is in accordance with relevant professional guidance and current legislation.
* Assess complex issues within an interdisciplinary framework.


Responsibilities

Qualifications

* Registered Nurse NMC in Mental Health, Social Work registered to Social Work England or Qualified Occupational Therapist with Membership of HCPC.
* Relevant teaching qualification.


Training, Experience & Knowledge

* Evidence of continued professional development.
* Understanding of the needs of older people with mental health problems and knowledge of assessing complex issues within an interdisciplinary framework.
* Minimum of two years experience in a clinical environment where a significant portion of clinical time is spent assessing or treating patients with dementia.
* Experience of working in General or Mental Hospital settings.
* Experience of providing mental health liaison support to staff caring for older people.
