The Orchard Medical Centre is a dynamic, friendly teaching practice based on the outskirts of Bristol and we are looking to recruit an experienced Nurse Practitioner working 30 hours per week spread over 3 working days, to join our existing friendly team within a dynamic and progressive primary care setting. The role includes autonomously managing a list of patients. You will be responsible for history taking, assessment, diagnosis and treatment of routine and same day presentations. Pay is depending upon level of experience/qualifications within the area. The ideal candidate will have experience in chronic disease and sexual health. You will supported by a team of friendly ANPS and GP's.

This post is only suitable to an experienced nurse practitiner who either prescribes or would be interested in undertaking non-medical prescribing.
Main duties of the job
This role is a challenging and diverse role where no 2 days are the same. It involves autonomously managing patients on a routine or acute same day list. This will include minor illness presentations, skin complaints, contraception. We would also be keen to see anyone who has experience in either a long term health condition such as asthma/diabetes or sexual health. As we are a teaching practice we are committed to providing training for the right candidate. You will be working as part of a team of 3 nurse practitioner's with guidance of a friendly team of GPs
The successful applicant should be flexible. There may also be the opportunity to work additional hours.

Job responsibilities
Tobe responsible for the delivery of nursing services, working as part of thepractice multidisciplinary team, delivering care within their scope of practiceto the entitled patient population. The nurse practitioner will be responsiblefor a number of clinical areas such as minor illness, health promotion, chronicdisease management, health prevention, as well as supporting the managementteam in the reviewing of clinical policy and procedure.
Thefollowing are the core responsibilities of the nurse practitioner. There may beon occasion, a requirement to carry out other tasks; this will be dependentupon factors such as workload and staffing levels:
a.Develop,implement and embed health promotion and well-being programmes
b.Providea triage service, actively managing or referring patients or treating patientswithin own scope of practice
c.Supportthe clinical team in the development of individual treatment plans for chronicdisease patients
d.Identify,manage and support patients as risk of developing long-term conditions,preventing adverse effects on the patients health
e.Provideroutine nursing care to patients as required in accordance with clinical basedevidence, NICE
f.Providewound care (ulcer etc.) to patients
g.Requestpathology services as necessary, processing and interpreting results asrequired
h.Referpatients directly to other services as appropriate
i.Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary / specialist care as required
j.Maintainaccurate clinical records in conjunction with extant legislation

a.Delegateclinical responsibilities appropriately (ensuring safe practice and the task iswithin the scope of practice of the individual)
b.Supportthe clinical team with all safeguarding matters, in accordance with local andnational policies
c.Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately
Deliver opportunistic health promotion where appropriate
Inaddition to the primary responsibilities, the nurse practitioner may berequested to:
a.Ifan independent prescriber, prescribe in a safe an effective manner, inaccordance with PGD and extant legislation
b.Actas the audit lead, effectively utilising the audit cycle
c.Monitorand ensure the safe storage, rotation and disposal of medicaments
d.Contributeto the implementation of an effective training programme for staff and studentswithin the practice
e.Provideclinical supervision for the nursing team
f.Participatein local initiatives to enhance service delivery and patient care
g.Supportand participate in shared learning within the practice
h. Continually reviewclinical practices, responding to national policies and initiatives whereappropriate
i. Be aware of andcontribute to the planning and implementation of CCG patient pathwayinitiatives
j. Participate in thereview of significant and near-miss events applying a structured approach root cause analysis (RCA).
