These roles are currently offered as a secondment from our system partners.
This important role is central to the delivery of CHC across Cornwall and Isles of Scilly with a primary objective of ensuring fair access to CHC with better outcomes and better experiences for our patients.
Our ideal candidate will be able to work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). Candidates will case manage clients to ensure that their needs are effectively met and promote Personal Health Budgets and personalisation, empowering people with complex health needs to live their best lives. You will be a highly motivated individual who is passionate about patient care and improving life opportunities.
Main duties of the job
You will promote good professional practice in CHC by advising and liaising with other health providers, care providers and Adult Social care colleagues. You will be accountable for managing a caseload of Continuing Healthcare assessments, reviews and placements which will include care planning, risk assessment and case management.
Successful candidates will be able to demonstrate the ability to remain calm, to handle emotive, complicated, and sensitive issues. You will need to show tact and diplomacy, good workload management skills and be able to negotiate with confidence and provide innovative solutions for complex care requirements.
As part of the county wide team you will work flexibly and responsively to the demands of the service to support the CHC process.You must be able to travel independently to visit individuals and attend meetings at various locations across Cornwall as part of your day-to-day work.
