Our Enhanced Care Support Workers are at the heart of providing direct patient centred care, supporting the nursing teams as well as mental health services and wider multi‑disciplinary teams. The role is varied, equally challenging and rewarding, where you will make a difference to the experience of our patients. The role includes:
Responsibilities
* Providing direct patient care including meeting hygiene, continence, nutrition, pressure area care and mobility needs
* Taking and recording patients’ vital signs, identifying and escalating changes to a patient’s condition
* Providing one‑to‑one observation/cohort care for vulnerable patients who present a high risk of harm
* Providing care and compassion, whilst demonstrating professional behaviour and utilising a range of appropriate communication skills
* Recognise and respond appropriately to the emotional needs of vulnerable patients
* Support the risk management of vulnerable patients by identifying, reporting and escalating issues of concern
* Support the management of challenging or disturbed behaviour in accordance with Trust policies and procedures
* Under the guidance of registered professionals assist with the therapeutic interventions, maintaining a person‑centred and holistic approach
* Support patients to engage in therapeutic activities which promote independence, resilience and well‑being
* Undertaking training and competencies related to the role competencies related to the specific department
* Escorting patients
* Reporting directly to the Trust’s Enhanced Care Lead; seeking people able to work a varied shift pattern including long days, nights and weekends, in a variety of settings including Urgent and Emergency Care, Older People Services, and Paediatrics
Qualifications
* Recent and significant UK experience as a health/enhanced care support worker
* Successfully achieved the Care Certificate
* Level 3 qualification preferably in a health and social care related subject
